Person-Centered Planning in Adult Day Services, Theme One: Assessment
Assessment
The first theme I’ll be exploring is the most simple: “Assessment”. I suggest that it is simple, because all centers engage in a formal assessment process with participants at least annually and informal assessments daily.
Person-Centered Planning HCBS Regulations that Address “Assessment”
The Person-Centered Service Plan must reflect the services and supports that are important for the individual to meet the needs identified through an assessment of functional needs, as well as what is important to the individual with regard to the preferences for the delivery of such services and supports.” Final Register, 4411.301 (c)(2)
“Reflect clinical and support needs as identified through an assessment of functional need.” Final Register, 4411.301 (c)(2)(iii)
How to Operationalize These Regulations:
- Engage the participant in a collaborative care planning process to elicit the participant’s goals
- Document these goals in the participant’s assessment
- Determine with participant the services and supports he or she already has in place, would like to have, and suggest services and supports that he or she may need to meet his or her goals
- Create policies and procedures for the assessment guidelines
Ensure Compliance
- Are the participant’s goals documented in the assessment?
- Does the assessment include an assessment of the participant’s functional needs?
- Are the services and supports provided outside of the center documented on the assessment?
- Are the services and supports needed to meet the participant’s goals reflected in the care plan?
- Do you have policies and procedures in place that define how this is to be implemented?
