*Please note, that these policies and procedures are an addition to your current policy and procedure manual, they do not cover all of your center’s policies and procedures. Policy and procedures include policies to meet the Federal HCBS requirements, CDA’s new survey tool, new participant rights and the new IPC.
**Forms included are assessment, reassessment, and quarterly forms for all disciplines and consultants. Also included are MDT forms, addendum to the participant agreement, letters and other forms needed to support person-centered planning requirements. These forms are meant to compliment other forms you may utilize in your center (flow sheets, logs, narrative notes, etc.). All forms are “fillable”, simplifying the processes and helping to ensure legibility. Form set corresponds to the new IPC in California as well.
Amanda Graham Sillars, MSW, LCSW, has been researching and developing manuals for Person-Centered Care and Planning since 2012. She, along with her business partner, Cordula Dick-Muelhke have developed person-centered training programs that have been attended by ADHC center staff, owners, CDA staff, and managed care organization employees. In August of this year, they were invited by CDA to provide a workshop for them on integrating the Federal HCBS Person Centered regulations with the current California Model of ADHC/CBAS services.
Amanda has been providing consulting services and writing ADHC/CBAS manuals help centers meet regulatory requirements and ensure effectiveness and efficiency for ADHC/CBAS centers since 1997. Person-Centered Planning requirements are a major change to the way that we care for our participants; they are complicated, detailed and can seem confusing. But they are effective in supporting both improved participant care and staff’s feelings of success. Amanda’s Person-Centered Policy and Procedures and Forms manuals were created to make the transition from current operations to Person-Centered Planning operations smooth and simple. Just follow the policies and complete the forms, and you will be well on your way to a great survey (and happier participants, families, and staff!)
