ALTERATION IN COMMUNICATION
Primarily with this problem, you want to mainly be concerned about participant’s emotions and safety. You will not be doing the work of the speech therapist, but often depression and a low frustration level are associated with language difficulties. Keep this in mind as you care plan for this one. * Remember—A different language rather than English is not considered a language barrier at the Center unless the Center does not have staff that speak this language and who can consistently translate.

Difficulty pronouncing words

Not being able to communicate needs
(On this care plan, you will focus your goals on the “resulting in” or “at risk for” statement, as the “evidenced by” statements are things that will most likely be handled by the speech therapist.)
INTERVENTIONS
- Ask Participant questions that require 1-2 word responses
- Encourage participant to pronounce words slowly, when speaking to participant
- Teach participant how to use communication board/book
- Do not cover mouth when speaking to participant.
- Make referral to community resource for the hearing impaired
- Touch participant prior to speaking
- Remove as much background noise when speaking to participant
- Encourage non-verbal communication when speaking with participant
- Disregard inappropriate words when participant is conversing
- Use short, direct phrases when talking to participant
- Speak slowly and clearly, raise volume and check for comprehension
- Do not rush participant when he or she is attempting to communicate
- Familiarize family/care giver with communication system
- Use non-verbal communication practices
GOAL
- Participant will have needs understood
- Participant will not be harmed due to communication problems
- Participant will exhibit increased understanding as evidenced by __________
- Participant will not experience increased frustration due to communication problem
- Participant will not be socially isolated due to communication problems
- Participant will receive a hearing evaluation
- Participant will wear hearing aids or express satisfaction with not wearing them (Remember, participant has the right to not wear the hearing aids if he or she does not wish.)
